UefiCpuPkg/MpInitLib: Add MP_ASSEMBLY_ADDRESS_MAP

In MpInitLibInitialize(), invoke AsmGetAddress() to get get assembly functions'
entry addresses and the sizes from returned MP_ASSEMBLY_ADDRESS_MAP structure.
